I defeated the npc w/ kick and fist (using knuckle duster) on first attempt but was told I failed, so I removed the knuckle duster and tried again using kick/fist only...once again...fail, unequipped all weapons and tried again....failed again. What did I do wrong????

You can't use any items during fight - mission will be failed in this case, and future attacks will not affect on the state.

To elaborate knuckle dusters are still melee weapons - you needed to hit using your (bare) fists.
